What services are available?
Families can request case advocacy, respite care, and/or support group. All three services may be provided at once, or families can select one or a combination of two.

How much does it cost?
Post permanency services are free to families and will be available as long as funding remains available.

What qualifies me to receive post permanency services?
Post permanency services are available to families living in Pennsylvania who have adopted or provided a child with Permanent Legal Custodianship. If finalized prior to the child turning 12 years of age, families have until the child turns 18 to receive services. If finalized after the child is 12, families can access services until the child turns 21 years old.

How do I obtain services?
Contact the SWAN helpline at 1-800-585-SWAN. A staff will walk you through the process.

How do I know when it is appropriate to request post permanency services?
Post permanency services are not intended to be used during crisis situations. Post permanency services were developed to support adoptive families who are struggling and need some extra assistance. Families can request these services at any time they feel they may need some extra guidance or support. It is best to request services as soon as the family begins experiencing challenges with the hope that the services can help the family avoid reaching a crisis situation.
