Home Study Document List

The following documents are necessary to complete the home study report:

1. Birth certificates of adoption applicant(s)
2. Marriage license
3. Divorce decree (if applicable)
4. Processed criminal clearance form(s)
5. Processed childline clearance form(s)
6. Letter(s) of employment stating years of service, title, salary and that you are an employee in good standing
7. Letter(s) from your doctor stating that you are in good physical and mental health with no parenting restrictions
8. Three years of recent federal tax returns (only the first two pages of the returns are required
9. *Completed and notarized financial form - supplied by the agency
10. Proof of health insurance coverage for the adopted child
11. *Completed and notarized Letter of Intent To Adopt, a sample letter will be provided by the agency
12. Three letters of reference from non-relatives

* All but item numbers 9 and 11 should be copies.
