Our PQI Structure, Stakeholders and Outcomes
Families United Network, Inc. has developed an ongoing structure of the PQI (Performance and Quality Improvement) plan which includes a “Plan, Do, Study, and Act” process. The Do is the intervention needed to carry out the plan. The Study is the evaluation of results. Last, the Act is using the outcomes to improve the plan development. This process creates an ongoing feedback loop and involves each level of the agency’s organizational chart. The formalized plan will evolve with ongoing improvements, as well as annual review and approval from the Board of Directors.
Our Stakeholders and Their Participation in the PQI Process - The participation of stakeholders is fundamental to the success of the PQI Process. The Families United Network agency stakeholders include: clients and families served by the agency; employees, volunteers, and consultants; members of the governing body and advisory boards community agencies/partners, vendors; and the county representatives in which we work.
There are many ways stakeholders participate in the PQI process. Information gathered from stakeholders is used to help define the organization’s mission and values, identify quality improvement goals, choose service outcomes, help set the agency’s long term direction, and review the organization’s overall performance in relation to established expectations. The PQI facilitator will annually review the use of stakeholders as well as the methods used to engage participation. Recommendations for change will be addressed during any long term planning meeting or the Board of Directors review of the annual PQI plan.
PQI Outcomes Measurement - Families United Network, Inc. has developed and outcomes measurement system in each of its programs which evaluate individual progress and program effectiveness. Key areas include the length of stay for clients in care and their discharge location upon leaving the program.
